Help shape palliative and end of life care in Bury

Health and social care organisations in Bury are currently working on a programme to improve services for adults who have palliative care needs or are in the last year of life and are asking patients and carers to help them shape these changes.

Palliative care refers to care provided to someone with a life limiting illness. End of life care usually refers to services provided to people in the last year of life.

The organisations, who are part of the Bury Integrated Delivery Collaborative, would like as many people as possible who are using these services, or have used them in the past six months, to complete a short, anonymous survey about their experiences.

They are also keen to learn about the experience of family members and carers of people who are in receipt of palliative and end of life care services.

Please click on the links and help the organisations learn about your experiences. The surveys will be live for two weeks.
